- Patron Information Form
- All patrons must print and fill out this form to gain access to materials located in the Billups-Garth Archives vault. Along with this form, one must present a valid photo identifiacation. The Patron Information Form need only be filled out once every year. Copies of the form are also available at the Local History Room.
